Federal, State and Local Tax Forms
The Library has not offered tax forms to our patrons for a number of years because of staffing considerations and the difficulty in obtaining a supply the forms our patrons actually need. Although our Federal and State Governments no longer send out forms, both the forms and the instruction booklets are available online. There are also local sources for the hardcopy forms as listed below.
Our staff is not qualified to provide tax advice or information. If you know the number of the Federal or State form you need, we will print the first copy for you at no charge. Additional copies are available at $.15 per page for black and white copies and $1.00 per page for color copies.

In addition, there is a local IRS office in Monroeville at 4314 Old William Penn Highway. The building is at the intersection of Old William Penn Highway and Duff Road (The Golden Corral Restaurant is at the top of Duff Road where it intersects with Route 22, also William Penn Highway.). The IRS tax office has a second floor entrance around the right hand side of the building. It is open from 8:30 a.m. till 4:30 p.m. Monday through Thursday and is closed for lunch from 12:30 till 1:30. The phone number to order forms is: 800-829-1040.

There is a local PA tax office in Greensburg at 15 West Third Street, second floor. Their phone number is 724-832-5386. We also refer patrons to the local office of our Pennsylvania State Senator, Don White, at 3950 William Penn Highway in Murrysville. The office is next to Dunkin' Donuts. We suggest that you phone 724-327-2422 before you go to the office to make sure there is someone there to assist you.
